At Mosaic Case Management and Therapies, we are a new and rapidly growing company providing specialist support to individuals with catastrophic injuries, neurological conditions, and complex care needs. 

As we continue to expand, we are looking to build a strong network of Associate Case Managers who share our commitment to delivering high‑quality, person‑centred care.

We are seeking skilled and experienced health and social care professionals with qualifications in areas such as occupational therapy, social work, nursing, physiotherapy, speech and language therapy, or clinical psychology. 

You will have substantial experience working within community settings and a strong understanding of coordinating and managing tailored care packages and rehabilitation programmes.

You must be organised, proactive, and confident in producing high‑quality assessments, progress updates, and professional reports. Experience working with individuals with complex injuries or conditions is essential.
